- Submit Waiver of Liability, Assumption of Risk and Indemnity Agreement
- Sign University of California Consent for Photo/Video/Recording Release
- Submit Disclosure of Critical Policy Information

As part of the on-boarding process, students are asked by UCLA Anderson Student Technology Services to complete an online Technology Orientation, which includes Anderson email and network accounts activation and laptop configuration.

EMBA students are automatically opted out of the UCLA Health Insurance for students. Therefore, providing immunizations and health records (aside from COVID-19 records) are not required as part of the admissions process for Executive MBA students. You may ignore the immunization requirement submission listed on myUCLA.

While immunizations are not required for EMBA program enrollment, the UCLA Health Arthur Ashe Student Health Center highly recommends that each student have the immunizations listed on the website here.
